Pre-discharge Stage
Pre-discharge stage (week 6 post-operatively in preparing the patient to return home)
Within this week, while continuing for intensive rehabilitation program, preparation is made for efficient & safe discharge. A home visit is planned & to be conduct on the middle of the week. At week 6, Mrs. Chan prefers to use the walker as she feels that her legs are still weak and painful. The team recommends Mrs. Chan to use the walking frame for 2 more weeks after discharge home. Then, she may be able to shift to use the cane.
